Job Title: LPN, Long-Term Care (LTC)
Job Summary:
As an LPN specializing in Long-Term Care, you will play a crucial role in providing compassionate and high-quality nursing care to our elderly residents. Working closely with registered nurses and other healthcare professionals, you will assist in the implementation and coordination of resident care plans, ensuring their physical, emotional, and social well-being. Your expertise and dedication will be essential in maintaining a safe and comfortable environment for our residents while promoting their independence and dignity.
Responsibilities and Duties:
- Administer and monitor medication, treatments, and procedures according to facility protocols and physicians' orders
- Collaborate with registered nurses and healthcare professionals to develop and implement individualized care plans for residents
- Provide direct nursing care, assistance, and support to residents with activities of daily living (ADLs), including hygiene, mobility, and nutrition
- Monitor and record vital signs, symptoms, and changes in residents' conditions, reporting to the supervising nurse or physician as necessary
- Implement infection control and safety measures, ensuring compliance with all regulatory and facility guidelines
- Foster a nurturing and supportive environment, establishing rapport with residents and their families to address concerns and provide emotional support
- Contribute to resident assessments, documenting and reporting any significant changes or incidents
- Assist in coordinating activities, therapies, and social events to promote residents' engagement and well-being
- Collaborate with the interdisciplinary healthcare team to ensure continuity and quality of care
- Stay updated with industry standards, best practices, and changes in healthcare regulations
Qualifications and Skills:
- Valid LPN license in the state of [State] (Must be in accordance with state nursing regulations)
- Completion of an accredited LPN program
- Proven experience working in a long-term care or similar healthcare setting is preferred
- Solid knowledge of nursing principles and practices, including infection control and safety protocols
