We are seeking a skilled and compassionate Long-Term Care Registered Nurse (LTC RN) to provide high-quality nursing care to residents in a long-term care or skilled nursing facility. The RN will be responsible for resident assessments, medication administration, care planning, clinical documentation, and coordination with physicians and interdisciplinary teams to ensure safe and effective care.
Provide direct nursing care to residents in a Long-Term Care (LTC) setting
Perform resident assessments, monitor vital signs, and document clinical changes
Administer medications, injections, IV therapy (if applicable), and treatments
Develop and update individualized care plans based on resident needs
Respond to emergencies, provide triage, and report changes to physicians
Supervise and guide LPNs and CNAs to ensure proper resident care
Maintain accurate nursing documentation (MAR, TAR, progress notes)
Ensure compliance with facility policies, state regulations, and infection control standards
Educate residents and families regarding care, medications, and treatment plans
Participate in admissions, discharges, and resident care conferences
Active Registered Nurse (RN) license (state-specific)
Previous experience in Long-Term Care / Skilled Nursing preferred
Strong clinical assessment and medication administration skills
Ability to work independently and in a team environment
Excellent communication, empathy, and time-management skills
Experience with wound care, dementia care, and geriatric nursing
Familiarity with EMR systems used in LTC facilities
BLS certification (ACLS a plus depending on facility)
Charge nurse or leadership experience
