Job title: Registered Nurse Home Health RN
Job Summary:
As a Registered Nurse Home Health RN, you will be responsible for providing skilled nursing care to patients in their homes. You will assess patient needs, develop care plans, administer treatments, and educate patients and their families on how to manage their conditions.
Responsibilities and Duties:
- Conduct initial and ongoing assessments of patients
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed by a physician
- Monitor patient progress and update care plans as needed
- Educate patients and their families on disease management and self-care techniques
- Coordinate care with other healthcare providers
- Document patient information and treatment plans accurately and timely
Qualifications and Skills:
- Valid RN license
- Minimum of 1 year of experience in home health or a related field
- Excellent communication and interpersonal skills
- Strong clinical assessment and critical thinking skills
- Ability to work independently and manage a caseload efficiently
- Knowledge of Medicare and Medicaid regulations
- BLS certification
- Valid driver's license and reliable transportation
