Job Title: Registered Nurse Home Health RN
Job Summary:
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our home health team. The RN will be responsible for providing quality healthcare services to patients in their homes. The ideal candidate will have strong clinical skills, excellent communication abilities, and a passion for delivering personalized care to patients.
Responsibilities and Duties:
- Conduct initial assessments of patients' health status
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed by physicians
- Educate patients and their families on managing their conditions
- Monitor patients' progress and adjust care plans as needed
- Collaborate with multidisciplinary healthcare team members
- Document all patient interactions and interventions accurately
- Provide emotional support to patients and families
Qualifications and Skills:
- Valid RN license in the state of practice
- Minimum of one year of nursing experience, preferably in home health
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work independently and as part of a team
- CPR certification
- Reliable transportation and valid driver's license
