Job Title: Inpatient Care Coordinator (Care Manager)
Job Summary:
The Inpatient Care Coordinator (Care Manager) is responsible for coordinating and managing the care of patients who are admitted to the hospital. They work closely with healthcare providers, patients, and their families to ensure that the patient's needs are met and that they receive appropriate care during their hospital stay.
Responsibilities and Duties:
1. Assess the patient's medical, social, and emotional needs upon admission to the hospital.
2. Develop and implement a care plan that addresses the patient's specific needs and goals.
3. Communicate and collaborate with healthcare providers, including physicians, nurses, therapists, and social workers, to coordinate the patient's care.
4. Advocate for the patient and ensure that they receive appropriate care and services.
5. Educate patients and their families about the patient's condition, treatment plan, and available resources.
6. Monitor the patient's progress during their hospital stay and make adjustments to the care plan as needed.
7. Coordinate discharge planning and ensure a smooth transition for the patient to their next level of care.
8. Document all interactions and interventions in the patient's medical record.
Qualifications and Skills:
1. Bachelor's degree in nursing, social work, or a related field.
2. Current licensure or certification in a healthcare-related field.
3. Previous experience in care coordination or case management.
4. Strong communication and interpersonal skills.
5. Ability to work effectively in a fast-paced healthcare environment.
6. Knowledge of medical terminology and healthcare systems.
7. Critical thinking and problem-solving skills.
8. Ability to work collaboratively with a multidisciplinary team.
