Job Title: Long-Term Care (LTC) Nurse
Job Summary:
As an LTC Nurse, you will provide compassionate and high-quality care to elderly or disabled individuals in long-term care facilities. You will be responsible for assessing residents' health needs, developing care plans, administering medications, and monitoring patient progress.
Responsibilities and Duties:
1. Conduct comprehensive assessments of residents' health status
2. Develop individualized care plans based on assessment findings
3. Administer medications and treatments as prescribed by healthcare providers
4. Monitor residents' progress and modify care plans as needed
5. Collaborate with interdisciplinary team members to ensure continuity of care
6. Provide emotional support and comfort to residents and their families
7. Educate residents and families on healthcare management and preventive care
8. Document all care provided in a timely and accurate manner
9. Adhere to all federal and state regulations governing LTC facilities
10. Participate in staff meetings and training sessions
Qualifications and Skills:
- Valid registered nurse (RN) license
- Previous experience in long-term care settings preferred
- Strong assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work effectively in a fast-paced environment
- Proficiency in electronic health record systems
- Compassionate and patient-centered approach to care
