Job Title: LPN, Long Term Care (LTC)
Job Summary:
As an LPN in Long Term Care (LTC), you will be responsible for providing quality nursing care to residents in long-term care facilities. Your primary role will be to assess patients' needs, develop care plans, administer medication, and collaborate with other healthcare professionals to ensure the well-being of residents.
Responsibilities and Duties:
- Conducting patient assessments and developing individualized care plans
- Administering medication and treatments as prescribed by healthcare providers
- Monitoring patients' overall health and well-being
- Collaborating with physicians, nurses, and other healthcare professionals to ensure comprehensive care
- Documenting patient progress and communicating with family members and caregivers
- Providing emotional support and assistance to residents and their families
- Implementing infection control protocols and ensuring a safe environment for residents
- Assisting with activities of daily living, such as bathing, dressing, and eating
Qualifications and Skills:
- Current LPN license in the state of practice
- Previous experience in long-term care settings is preferred
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
- Proficiency in basic nursing skills and medical terminology
- Compassionate and patient-centered approach to care
