Job Title: Social Worker
Job Summary:
As a Social Worker, you will be responsible for providing support and assistance to individuals and families facing various challenges and crises in their lives. You will work with clients to assess their needs, develop action plans, and connect them to resources and services in the community.
Responsibilities and Duties:
1. Conduct assessments to determine clients' needs and develop individualized service plans.
2. Provide counseling and emotional support to clients experiencing mental health issues, substance abuse, domestic violence, homelessness, or other challenges.
3. Connect clients to relevant social services, organizations, and community resources.
4. Advocate on behalf of clients to ensure they receive the necessary support and services.
5. Monitor clients' progress and make adjustments to service plans as needed.
6. Collaborate with other social service agencies, healthcare providers, and community organizations to coordinate care for clients.
7. Maintain accurate and confidential client records.
8. Participate in team meetings and case conferences to discuss client progress and collaborate on treatment plans.
9. Stay current on social work best practices and ethical standards.
10. Provide crisis intervention and support to clients in emergency situations.
Qualifications and Skills:
- Bachelor's or Master's degree in Social Work or related field.
- State licensure or certification as a Social Worker.
- Strong interpersonal and communication skills.
- Ability to work with diverse populations and cultural backgrounds.
- Knowledge of community resources and social service programs.
- Experience in case management, counseling, and crisis intervention.
- Empathy, compassion, and a nonjudgmental attitude towards clients.
- Strong organizational and time management skills.
