Job Title: MDS Coordinator
Job Summary:
We are seeking a detail-oriented and highly organized MDS Coordinator to join our team. The MDS Coordinator will be responsible for coordinating and completing the Minimum Data Set (MDS) assessments for all residents in our facility. This role requires a strong understanding of long-term care regulations and guidelines, as well as excellent communication skills to work with healthcare professionals and residents.
Responsibilities and Duties:
- Coordinate and complete MDS assessments for all residents according to federal and state regulations
- Collaborate with healthcare professionals to gather necessary information for accurate assessments
- Ensure accuracy and timeliness of MDS submissions to meet regulatory deadlines
- Develop and implement care plans based on MDS assessments
- Monitor resident progress and update care plans as needed
- Educate staff on MDS processes and regulations
- Participate in meetings with interdisciplinary team to discuss resident care and progress
Qualifications and Skills:
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a valid license
- Minimum of 2 years of experience in long-term care or skilled nursing facility
- Strong understanding of MDS assessments and regulations
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficient in computer software for data entry and documentation
