Job Title: Inpatient Care Coordinator
Job Summary:
The Inpatient Care Coordinator is responsible for overseeing and coordinating the care of patients during their stay in a healthcare facility. They work closely with physicians, nurses, and other healthcare professionals to ensure that patients receive the appropriate treatment and services to promote their health and well-being.
Responsibilities and Duties:
- Coordinate and facilitate the admission and discharge process for patients
- Develop and implement care plans based on individual patient needs
- Communicate with healthcare team members to ensure continuity of care
- Collaborate with insurance companies to authorize services and treatments
- Monitor and evaluate patient progress and make recommendations for adjustments to care plans
- Provide emotional support and guidance to patients and their families
- Educate patients on their condition and treatment options
- Maintain accurate and confidential patient records
Qualifications and Skills:
- Bachelor's degree in Nursing or related field
- Valid RN license
- Previous experience in care coordination or case management
- Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Knowledge of medical terminology and healthcare regulations
- BLS certification
- Proficiency in Microsoft Office Suite
