Job Title: Registered Nurse (RN) - Home Hospice RN
Job Summary:
The Registered Nurse (RN) - Home Hospice RN will provide compassionate and quality care to patients who are receiving end-of-life care in their homes. The RN will work closely with patients, families, and interdisciplinary teams to ensure comfort and dignity during this difficult time.
Responsibilities and Duties:
1. Conduct comprehensive assessments of patients' needs and develop individualized care plans.
2. Administer medications and treatments as prescribed by physicians.
3. Provide emotional support and education to patients and families about end-of-life care.
4. Coordinate care with other healthcare professionals, including social workers, chaplains, and home health aides.
5. Monitor and evaluate patients' condition and adjust care plans as needed.
6. Ensure compliance with regulatory requirements and documentation standards.
7. Participate in interdisciplinary team meetings to discuss patient care and treatment plans.
8. Communicate effectively with patients, families, and healthcare providers.
Qualifications and Skills:
1. Valid Registered Nurse (RN) license in the state of practice.
2. Minimum of 2 years of nursing experience, preferably in hospice care.
3. BSN degree preferred.
4. Excellent communication and interpersonal skills.
5. Compassionate and empathetic attitude.
6. Ability to work independently and as part of a team.
7. Strong critical thinking and decision-making skills.
8. Ability to prioritize and manage multiple tasks effectively.
