Job Title: Skilled Nursing (SNF) LPN in Long Term Care (LTC)
Job Summary:
As a Skilled Nursing LPN in Long Term Care, you will be responsible for providing direct patient care to residents in a skilled nursing facility. You will work under the supervision of registered nurses and physicians to implement and coordinate resident care plans, administer medications, and monitor vital signs. Your primary goal will be to ensure the highest level of care and support for residents in a long-term care setting.
Responsibilities and Duties:
1. Administer medications and treatments as prescribed by healthcare providers.
2. Monitor resident vital signs and report any abnormalities to the registered nurse.
3. Assist residents with activities of daily living, including bathing, dressing, and feeding.
4. Collaborate with interdisciplinary team members to develop and implement resident care plans.
5. Document resident observations and interventions in medical records.
6. Provide emotional support and companionship to residents.
7. Communicate effectively with residents, families, and healthcare team members.
8. Adhere to facility policies and procedures to ensure resident safety and well-being.
9. Participate in educational programs and continuing education opportunities to maintain licensure and stay current on best practices in nursing care.
Qualifications and Skills:
1. Graduation from an accredited LPN program.
2. Current LPN license in the state of practice.
3. CPR certification.
4. Minimum of 1 year of experience in long-term care or skilled nursing facility.
5. Strong communication and interpersonal skills.
6. Ability to work independently and as part of a team.
7. Knowledge of nursing principles and practices.
8. Excellent organizational and time management skills.
9. Compassionate and caring attitude towards residents.
