Job Title: Social Worker - Long Term Care
Job Summary:
The Social Worker will provide psychosocial support to residents and their families in long-term care facilities. They will assess the emotional, social, and financial needs of residents and develop treatment plans to address these needs. The Social Worker will collaborate with interdisciplinary teams to ensure the overall well-being of residents in long-term care settings.
Responsibilities and Duties:
1. Conduct psychosocial assessments of residents to identify their needs and preferences.
2. Develop individualized care plans to address social, emotional, and financial concerns.
3. Provide counseling and support to residents and their families to help them cope with the challenges of long-term care.
4. Coordinate with community resources to connect residents with services and support systems.
5. Advocate for residents' rights and assist them in navigating the healthcare system.
6. Collaborate with healthcare professionals, including nurses and therapists, to ensure comprehensive care for residents.
7. Document and maintain accurate and up-to-date records of residents' progress.
8. Participate in case conferences and interdisciplinary team meetings to contribute to the overall care plan.
9. Stay current on policies and regulations relevant to long-term care facilities.
10. Provide education and support to staff on social work issues related to long-term care.
Qualifications and Skills:
1. Master's degree in Social Work from an accredited program.
2. Licensed Clinical Social Worker (LCSW) preferred.
3. Experience working with older adults and individuals with chronic illnesses.
4. Knowledge of community resources and support services for long-term care residents.
5. Strong communication and interpersonal skills.
6. Ability to work independently and as part of a team.
7. Compassionate and empathetic attitude towards residents and their families.
8. Organizational skills and attention to detail.
9. Ability to handle difficult and emotionally charged situations.
10. Familiarity with electronic health record systems.
