Job Title: Social Worker - Long Term Care

Job Summary:

A Social Worker in Long Term Care is responsible for providing support and assistance to elderly patients and their families in long-term care facilities. They help residents cope with the challenges of aging, illness, and disability, and work to improve their overall quality of life.

Responsibilities and Duties:

- Conduct assessments of patients' emotional, social, and environmental needs

- Develop and implement care plans to address patients' concerns and goals

- Coordinate with healthcare providers and community resources to ensure continuity of care

- Provide counseling and support to residents and their families

- Advocate for patients' rights and access to services

- Assist with discharge planning and transition to alternative care settings

- Maintain accurate and up-to-date documentation of patient interactions and progress

Qualifications and Skills:

- Bachelor's or Master's degree in Social Work or related field

- State licensure as a Social Worker

- Experience working with elderly populations in long-term care settings

- Strong communication and interpersonal skills

- Ability to work as part of a multi-disciplinary team

- Knowledge of relevant laws and regulations related to long-term care
