Job Title: Social Worker - Long Term Care
Job Summary:
A Social Worker in Long Term Care is responsible for providing support and assistance to elderly patients and their families in long-term care facilities. They help residents cope with the challenges of aging, illness, and disability, and work to improve their overall quality of life.
Responsibilities and Duties:
- Conduct assessments of patients' emotional, social, and environmental needs
- Develop and implement care plans to address patients' concerns and goals

- Provide counseling and support to residents and their families

Qualifications and Skills:
- Bachelor's or Master's degree in Social Work or related field
- State licensure as a Social Worker
