Job Title: Registered Nurse - Care Manager
Job Summary:
We are seeking a dedicated Registered Nurse to join our team as a Care Manager. The Care Manager will be responsible for coordinating and managing the care of patients, collaborating with healthcare providers, and ensuring quality and efficient care delivery. This role requires strong clinical knowledge, excellent communication skills, and a compassionate approach to patient care.
Responsibilities and Duties:
- Develop and implement care plans for patients in collaboration with healthcare providers
- Conduct assessments and evaluate patient progress
- Communicate with patients, families, and healthcare team members to ensure coordinated care
- Advocate for patients and provide support throughout their healthcare journey
- Monitor and manage patient referrals and transitions of care
- Educate patients on their conditions and treatment options
- Document patient information accurately and maintain confidentiality
- Stay abreast of current healthcare practices and regulatory requirements
Qualifications and Skills:
- Current RN license in the state of practice
- Bachelor's degree in Nursing (BSN) preferred
- Minimum of 2 years of clinical nursing experience
- Experience in care management or care coordination preferred
- Strong communication and interpersonal skills
- Ability to prioritize and multitask in a fast-paced environment
- Knowledge of healthcare regulations and quality improvement processes
