Job title: Travel Registered Nurse (RN) - Home Health/Case Manager
Job Summary:
We are seeking a compassionate and experienced Registered Nurse to join our team as a Travel RN focusing on Home Health and Case Management. In this role, you will be responsible for providing high-quality care to patients in their homes, assessing their needs and coordinating their care plans.
Responsibilities and Duties:
- Conducting comprehensive assessments of patients in their homes
- Developing individualized care plans based on assessment findings
- Implementing and overseeing medical treatments and interventions
- Communicating with physicians and other healthcare professionals to coordinate patient care
- Educating patients and their families on health management techniques
- Documenting patient progress and updating care plans as needed
- Providing emotional support to patients and families during challenging times
- Adhering to all legal and ethical standards in healthcare delivery
Qualifications and Skills:
- Active RN license in the state of practice
- Minimum of 2 years of nursing experience, preferably in home health or case management
- BLS certification
- Strong communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Excellent organizational and time management skills
- Willingness to travel to different locations as needed
