Job title: LVN - Skilled Nursing (SNF)
Job Summary:
The LVN - Skilled Nursing (SNF) is responsible for providing nursing care and support to residents in a skilled nursing facility. They work under the supervision of registered nurses and provide direct patient care including administering medications, wound care, and monitoring patients' vital signs.
Responsibilities and Duties:
1. Administer medications and treatments as prescribed by physicians
2. Monitor and record patients' vital signs
3. Assist with activities of daily living, including bathing, dressing, and feeding
4. Provide wound care and dressings as needed
5. Communicate with residents' families and healthcare team members
6. Follow facility policies and procedures to ensure quality care for residents
7. Document all patient care activities accurately and timely
8. Collaborate with registered nurses and other healthcare professionals to develop care plans for residents
9. Follow infection control protocols to prevent the spread of illness
10. Participate in continuing education and training programs to stay current on best practices in nursing care
Qualifications and Skills:
1. Licensed Vocational Nurse (LVN) in the state of practice
2. Minimum of 1 year of experience in skilled nursing or long-term care setting
3. CPR certification
4. Ability to communicate effectively with residents, families, and healthcare team members
5. Strong attention to detail and ability to follow instructions accurately
6. Compassionate and patient-centered approach to care
7. Ability to work independently and as part of a team
8. Knowledge of nursing standards and regulations
9. Strong clinical skills and understanding of basic medical terminology
10. Ability to prioritize and manage multiple tasks efficiently
