The Registered Nurse (RN) in a Skilled Nursing Facility (SNF) or Long Term Care (LTC) setting delivers comprehensive nursing care to residents with chronic conditions, post-acute needs, and long-term health requirements. The RN ensures high-quality, patient-centered care while maintaining regulatory compliance and supporting residents’ overall well-being.
Conduct comprehensive resident assessments and ongoing evaluations
Develop, implement, and update individualized care plans
Administer medications, treatments, and wound care as prescribed
Monitor resident conditions and respond to changes in health status
Coordinate care with physicians, therapists, and interdisciplinary teams
Supervise and support LPNs, CNAs, and nursing staff
Ensure accurate and timely documentation in EMR systems
Educate residents and families on care plans and health management
Maintain compliance with infection control, safety, and regulatory standards
Active Registered Nurse (RN) license
Experience in Skilled Nursing Facility or Long Term Care preferred
Strong clinical assessment and critical thinking skills
Knowledge of geriatric care, chronic disease management, and wound care
Excellent communication and leadership abilities
Ability to manage multiple residents in a fast-paced environment
Familiarity with EMR documentation and healthcare regulations
