Position Title: Long Term Care (LTC) RN

Job Summary:
The Long Term Care (LTC) Registered Nurse (RN) provides comprehensive nursing care to residents in long-term care facilities such as nursing homes and rehabilitation centers. The RN ensures high-quality patient care, supports daily living needs, and promotes residents’ overall well-being.

Key Responsibilities:

Assess residents’ physical and mental health conditions
Develop and implement individualized care plans
Administer medications and treatments as prescribed
Monitor vital signs and document clinical observations
Supervise LPNs and CNAs to ensure quality patient care
Coordinate with physicians, therapists, and families
Respond to medical emergencies and changes in condition
Ensure compliance with healthcare regulations and safety standards
Provide education and emotional support to residents and families

Required Qualifications:

Active Registered Nurse (RN) license
BLS certification (ACLS preferred)
Experience in long-term care, geriatrics, or skilled nursing preferred
Strong clinical assessment and documentation skills
Excellent communication and leadership abilities
