Job Title: LPN – Skilled Nursing (SNF) / Med-Surg Adult

Primary Skill: Practical Nursing

Job Summary:

The Licensed Practical Nurse (LPN) in a Skilled Nursing Facility (SNF) / Med-Surg Adult setting provides direct patient care under the supervision of Registered Nurses and physicians. This role focuses on supporting adult and geriatric patients with medical-surgical conditions, ensuring quality care, comfort, and recovery in a long-term or post-acute care environment.

Key Responsibilities:

• Provide direct nursing care to adult and geriatric patients

• Monitor vital signs, symptoms, and patient conditions

• Administer medications, injections, and treatments as prescribed

• Perform wound care, dressing changes, and basic procedures

• Assist with activities of daily living (ADLs) such as bathing, dressing, and mobility

• Support post-surgical and chronic care patients

• Observe and report changes in patient condition to RN/physician

• Maintain accurate documentation in EMR systems

• Ensure infection control and patient safety protocols

• Collaborate with healthcare teams for coordinated care

Required Qualifications:

• Diploma in Practical Nursing (LPN)

• Active LPN license

• BLS (Basic Life Support) certification

• 1–3 years of experience in SNF, long-term care, or Med-Surg (preferred)

Work Environment:

• Skilled Nursing Facility (SNF) / Long-term care center

• Rotational shifts, including nights and weekends

Preferred Experience:

• Experience in SNF, rehabilitation, or Med-Surg units

• Familiarity with EMR systems

• Experience with post-operative and chronic care patients
