The Licensed Practical Nurse (LPN) in a Skilled Nursing Facility (SNF) / Med-Surg Adult setting provides direct patient care under the supervision of Registered Nurses and physicians. This role focuses on supporting adult and geriatric patients with medical-surgical conditions, ensuring quality care, comfort, and recovery in a long-term or post-acute care environment.
Provide direct nursing care to adult and geriatric patients
Monitor vital signs, symptoms, and patient conditions
Administer medications, injections, and treatments as prescribed
Perform wound care, dressing changes, and basic procedures
Assist with activities of daily living (ADLs) such as bathing, dressing, and mobility
Support post-surgical and chronic care patients
Observe and report changes in patient condition to RN/physician
Maintain accurate documentation in EMR systems
Ensure infection control and patient safety protocols
Collaborate with healthcare teams for coordinated care
Diploma in Practical Nursing (LPN)
Active LPN license
BLS (Basic Life Support) certification
1–3 years of experience in SNF, long-term care, or Med-Surg (preferred)
Patient care and clinical support
Knowledge of Med-Surg and geriatric care
Medication administration
Wound care and basic procedures
Communication and teamwork
Attention to detail and documentation
Skilled Nursing Facility (SNF) / Long-term care center
Med-Surg or post-acute care units
Rotational shifts, including nights and weekends
Experience in SNF, rehabilitation, or Med-Surg units
Familiarity with EMR systems
Experience with post-operative and chronic care patients
