The Weekend Field Registered Nurse (RN) provides compassionate, high-quality nursing care to patients in home and community settings. This role focuses on wound care management, hospice support, geriatric care, and overall patient well-being, ensuring continuity of care during weekends.
Provide in-home nursing care to patients with acute and chronic conditions
Perform advanced wound care, dressing changes, and infection prevention
Deliver hospice and palliative care with a patient-centered approach
Assess patient health status and develop individualized care plans
Administer medications, injections, and IV therapies as prescribed
Monitor patient progress and report changes to physicians and care teams
Educate patients and families on care plans, medications, and disease management
Maintain accurate documentation in EMR systems
Ensure compliance with healthcare standards and safety protocols
Coordinate with interdisciplinary teams for continuity of care
Bachelor’s Degree or Diploma in Nursing
Active Registered Nurse (RN) license
BLS (Basic Life Support) certification
Valid driver’s license and ability to travel for home visits
2–5 years of experience in home health, hospice, or wound care
Wound care and infection management
Hospice and palliative care knowledge
Geriatric patient care
Strong assessment and clinical judgment
Communication and patient education
Time management and independent working ability
Patient homes / community settings
Weekend shifts (Saturday & Sunday coverage)
Travel-based role with flexible scheduling
Certification in wound care (WCC or equivalent)
Experience in hospice or palliative care
Familiarity with EMR systems
