🏥 Job Title: Home Health RN / Hospice RN / RN Case Manager

📌 Job Summary

The Registered Nurse (RN) is responsible for delivering patient-centered care across home health, hospice, and case management settings. This role includes assessing patient needs, coordinating care plans, providing treatment, and ensuring comfort and quality outcomes, especially for patients requiring ongoing or end-of-life care.

🔑 Key Responsibilities

• Assess patient conditions and develop individualized care plans
• Provide in-home nursing care and monitor patient health status
• Administer medications and treatments as prescribed
• Manage pain and symptoms, especially in hospice care
• Coordinate care with physicians, therapists, and healthcare teams
• Educate patients and families on treatment plans and home care
• Ensure emotional support and comfort for patients and families
• Track patient progress and update care plans accordingly
• Maintain accurate documentation and compliance with healthcare standards

🎓 Required Qualifications

• Valid Registered Nurse (RN) License
• BLS/ACLS Certification (preferred)
• Bachelor’s Degree in Nursing (BSN) preferred

💼 Experience Required

• 1–3+ years of nursing experience
• Experience in Home Health / Hospice / Case Management preferred
