Job Title: LVN – Skilled Nursing Facility (SNF)

📌 Job Summary

The Licensed Vocational Nurse (LVN) in a Skilled Nursing Facility (SNF) provides direct patient care under the supervision of Registered Nurses (RNs) and physicians. This role focuses on supporting long-term care patients, administering treatments, and ensuring comfort and well-being.

🔑 Key Responsibilities

• Provide basic nursing care to patients in a skilled nursing facility
• Monitor and record vital signs and patient conditions
• Administer medications and treatments as prescribed
• Assist with wound care, IV therapy (as permitted), and procedures
• Support patients with daily living activities (ADLs)
• Observe and report changes in patient health to RNs/physicians
• Maintain accurate documentation and patient records
• Ensure compliance with safety and infection control protocols
• Collaborate with healthcare teams for patient care planning

🎓 Required Qualifications

• Valid LVN (Licensed Vocational Nurse) license
• Diploma or certification in practical/vocational nursing
• CPR/BLS Certification (preferred)

💼 Experience Required

• 1–2+ years of experience in skilled nursing or long-term care (preferred)
