π Job Summary
The Licensed Vocational Nurse (LVN) in a Skilled Nursing Facility (SNF) provides direct patient care under the supervision of Registered Nurses (RNs) and physicians. This role focuses on supporting long-term care patients, administering treatments, and ensuring comfort and well-being.
π Key Responsibilities

Provide basic nursing care to patients in a skilled nursing facility

Monitor and record vital signs and patient conditions

Administer medications and treatments as prescribed

Assist with wound care, IV therapy (as permitted), and procedures

Support patients with daily living activities (ADLs)

Observe and report changes in patient health to RNs/physicians

Maintain accurate documentation and patient records

Ensure compliance with safety and infection control protocols

Collaborate with healthcare teams for patient care planning
π Required Qualifications

Valid LVN (Licensed Vocational Nurse) license

Diploma or certification in practical/vocational nursing

CPR/BLS Certification (preferred)
