Job Title: Registered Nurse Care Manager, Home Health RN In Barre, VT
Job Summary: We are seeking a passionate Registered Nurse Care Manager, Home Health RN in Barre, VT, to join our team. The ideal candidate will be responsible for providing personalized care to patients in their homes and will manage and coordinate the care of a caseload of patients. The Registered Nurse Care Manager will work closely with patients, their families, and other healthcare professionals to ensure that each patient receives the best possible care.
Responsibilities and Duties:
- Develop individualized care plans based on patient needs and goals
- Implement and manage care plans using evidence-based practices
- Conduct in-home assessments to determine patients’ health status and needs
- Provide education to patients and families on disease management and prevention
- Coordinate care with other healthcare professionals and community resources
- Provide skilled nursing care, including wound care, medication management, and patient education
- Monitor and evaluate patient progress and adjust care plans as needed
- Maintain accurate and complete patient documentation
- Adhere to all safety and infection control policies and procedures
Qualifications and Skills:
- Current RN license in Vermont
- Minimum of 1 year of experience in home health care or a related field
- Excellent communication and interpersonal skills
- Strong critical thinking and problem-solving abilities
- Ability to work independently, manage multiple priorities, and meet deadlines
- Computer literacy, including proficiency with electronic medical records
