We are seeking a compassionate and experienced Licensed Independent Social Worker (LISW) to provide clinical social work services, mental health assessments, crisis intervention, and ongoing therapy support to individuals, families, and communities. The LISW will work collaboratively with healthcare providers, case managers, and community resources to ensure high-quality client care and improved outcomes.
Conduct biopsychosocial assessments, mental health evaluations, and risk screenings
Provide individual, family, and group counseling using evidence-based practices
Develop and manage treatment plans aligned with client goals
Offer crisis intervention, safety planning, and referrals when needed
Maintain accurate clinical documentation in compliance with state and organizational standards
Coordinate with multidisciplinary teams (psychiatrists, nurses, primary care, therapists)
Provide case management services and connect clients with community resources
Ensure ethical practice and compliance with HIPAA and licensing regulations
Support discharge planning, aftercare, and continuity of care
Participate in supervision, training, and professional development activities
Active LISW License (state-specific)
Master’s Degree in Social Work (MSW) from an accredited program
Strong knowledge of mental health conditions, trauma-informed care, and clinical interventions
Experience with documentation, treatment planning, and client progress tracking
Excellent communication, empathy, and problem-solving skills
Experience in behavioral health, hospital, outpatient, or community-based settings
Knowledge of CBT, DBT, Motivational Interviewing, and trauma-focused therapy
Familiarity with EMR/EHR systems
Bilingual skills are a plus
