Job Title: LPN LTAC (Licensed Practical Nurse - Long Term Acute Care)
Job Summary:
We are seeking a dedicated and compassionate LPN to join our team in a Long Term Acute Care (LTAC) facility. The LPN will be responsible for providing nursing care to patients with complex medical conditions requiring long-term care. This role requires a strong knowledge of nursing principles and the ability to deliver high-quality care in a fast-paced environment.
Responsibilities and Duties:
- Administer medications and treatments as prescribed by physicians
- Monitor and assess patient's condition and report any changes to the medical team
- Collaborate with other healthcare professionals to develop and implement care plans
- Assist with patient assessments, admissions, and discharges
- Provide emotional support and education to patients and their families
- Maintain accurate and detailed patient records
Qualifications and Skills:
- Current LPN license in the state of practice
- Minimum of 1 year of experience in long-term acute care or a similar setting
- BLS certification
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
- Excellent attention to detail and critical thinking skills
