Job Title: Registered Nurse (RN) Case Manager / Clinical Care Manager
Job Summary:
We are seeking a compassionate and skilled Registered Nurse (RN) Case Manager/Clinical Care Manager to join our healthcare team. The primary responsibility of this role is to oversee and coordinate the care of patients, ensuring they receive high-quality and timely medical services. The RN Case Manager/Clinical Care Manager will work closely with healthcare providers, patients, and their families to create individualized care plans and monitor the patient's progress.
Responsibilities and Duties:
- Conduct initial assessments of patients to determine their healthcare needs
- Develop and implement personalized care plans for patients
- Collaborate with healthcare providers to coordinate services and treatments
- Monitor patient progress and adjust care plans as needed
- Educate patients and their families on medical conditions and treatment options
- Advocate for patients to ensure they receive appropriate care
- Maintain accurate and up-to-date patient records
Qualifications and Skills:
- Registered Nurse (RN) license
- Bachelor's degree in nursing or related field
- Proven experience in case management or clinical care management
- Strong communication and interpersonal skills
- Ability to work effectively in a team and independently
- Knowledge of healthcare regulations and best practices
- Excellent organizational and time-management skills
