Job Summary:
The Registered Nurse (RN) Community Health Nurse is responsible for providing comprehensive healthcare services to individuals, families, and communities in various community settings. They promote health education and preventive care, monitor patient progress, and collaborate with other healthcare professionals to ensure the well-being of the community.
Responsibilities and Duties:
1. Conduct health assessments and screenings in the community
2. Develop and implement individualized care plans for patients
3. Provide patient education on disease prevention and management
4. Administer medications and treatments as prescribed
5. Coordinate care with other healthcare providers and community resources
6. Advocate for the health needs of the community
7. Evaluate and document patient outcomes
8. Participate in community outreach programs and health fairs
9. Stay current on healthcare trends and best practices
10. Maintain confidentiality of patient information
Qualifications and Skills:
1. Valid RN license
2. Bachelor's degree in Nursing
3. Previous experience in community health nursing preferred
4. Excellent communication and interpersonal skills
5. Strong assessment and critical thinking abilities
6. Ability to work independently and as part of a team
7. Knowledge of community resources and healthcare services
8. Proficient in electronic health records
9. CPR certification
10. Ability to travel to different community locations
