Job Title: Registered Nurse (RN) - Home Health
Job Summary:
We are seeking a compassionate and experienced Registered Nurse (RN) to join our home health team. The RN will be responsible for providing high-quality, in-home nursing care to patients in need. The ideal candidate will have excellent clinical skills, strong communication abilities, and a dedication to improving the lives of patients in their homes.
Responsibilities and Duties:
- Conduct in-home assessments of patients' health status
- Develop and implement individualized care plans
- Perform skilled nursing treatments and procedures
- Monitor and evaluate patient progress
- Educate patients and families on disease management and self-care techniques
- Collaborate with other healthcare professionals to ensure coordinated care
- Document patient assessments, interventions, and outcomes accurately and timely
- Adhere to all applicable laws, regulations, and professional standards
Qualifications and Skills:
- Current RN license in the state of practice
- Minimum of 2 years of nursing experience, with at least 1 year in home health preferred
- Excellent clinical assessment and critical thinking skills
- Strong communication and interpersonal abilities
- Ability to work independently and as part of a team
- Proficiency in electronic medical records (EMR) systems
- BLS certification
- Reliable transportation and valid driver's license
